* We work closely with a mobile lab for needs such as UA/ culture or blood tests
* We work closely with mobile diagnostic companies for Xrays and in home venous dopplers to avoid extra trips to a facility
* We assist lower income patients set up diaper/pads/chucks delivery to the home at no cost
* We work with a DME company to acquire appropriate splints and braces appropriate for patient needs such as LSOs and wrist splints as well as diabetic shoes
* We also work with Blue Moon Senior Counseling to provide seniors in home Licensed Clinical Social Workers for anxiety or depression
* Our clinicians are MoCA CERTIFIED and can provide your physician with an accurate cognitive assessment or just to have as a baseline
* Our clinicians are able to acquire a handicap placard for your car in the state of Illinois of you qualify
* Also, exercise equipment is issued to the patient for more consistent follow through with exercise programs
We appreciate you but would refer you to a home health agency if you are in need of a higher level service such as needing a nurse. .
This option often allows the patient more frequent visits than home health allows under the PDGM/ Home Health guidelines and gives them the option to feel safe in their home all while saving Medicare dollars at a lower cost of service.
